PERSONAL RESPONSIBILITY WAIVER

I am signing up knowing that I am fully responsible for my own safety and well-being.

By checking this box, I understand that Shelby Leigh and/or all persons associated with hosting this event are not responsible for my personal health, safety, or wellbeing. I understand there is inherent risk involved when I choose to participate in physical or wellness-related activity and travel. My participation is entirely voluntary and I understand that I have the right not to participate in any event at any time. I assume all risk of bodily injury, or illness that could occur mentally, emotionally, or physically. 

I hereby waive to the fullest extent permitted by law, the right to bring any suit, action or claim of any kind against Shelby Leigh LLC or any persons acting on behalf of the company, as a result of my participation in The Soulful Retreat.

I hereby indemnify and hold harmless Shelby Leigh LLC and all parties acting on their behalf from any liability arising from accident, injury, damages, or loss arising in any way connected to my participation in The Soulful Retreat. 

I agree not to copy, record, or broadcast in any manner any portion of the material within this retreat offered by Shelby Leigh LLC or any others acting on the company's behalf within The Soulful Retreat.
